YOUR FIRST VISIT

Welcome to Encompass Physical Therapy! Thank you for selecting our practice to take care of your physical therapy needs. We have an exceptional team of therapists and staff committed to understanding your needs and providing you with the highest standard of care.

What to expect on your first visit:

Your appointment will last about 45 minutes and will be dedicated to listening to you and fully understanding your condition. The therapists will diagnose your condition and develop a specific treatment plan that will enable you to return to a normal functional status.

What to bring:

Please bring with you the completed medical history, patient policies and cancellation policy, any pertinent paperwork from your doctor’s office, prescription/referral if required (**REQUIRED FOR ALL MEDICARE PATIENTS**), your insurance card and photo ID. Co-pays, co-insurance and/or deductible payments are due at the time of service. We accept Visa, MasterCard, Discover, cash or check.

What to wear:

Please wear clothes that will allow access to the part of the body that needs treatment. If you are being treated in regard to your upper body, we ask that you bring or wear a tank top or a loose-fitting short-sleeved shirt. If you are being treated in regard to any part of your back, we ask that you bring or wear shorts and a T-shirt. Sport bras are fine for our female patients. For your convenience, we have a changing room and lockers to secure your belongings.

Appointments:

Treatment sessions are by appointment only. Your appointment is dedicated to you and only you. In order to provide consistency, appointments are scheduled a month in advance to ensure getting a timeframe that is most convenient for you. Your appointments will be scheduled with the same therapist throughout your entire treatment here at Encompass Physical Therapy.

We make every effort to see our patients on time and respect their schedules; therefore, we request the same from our patients. We would appreciate you arriving five minutes earlier than your scheduled appointment. Please check in at the front desk, use the restroom or change if necessary and then have a seat in the waiting area until you are called by your therapist. If you arrive more than 15 minutes late for your appointment, we reserve the right to cancel your appointment, as we will not have adequate time for the treatment of your condition, and to avoid inconveniencing others.

Cancellations:

Please bear in mind our services are in high demand and we kindly ask for 24 hours’ notice should you need to cancel your appointment. Because it is very difficult to fill an open physical therapy appointment at the last minute, we would appreciate 24 hours for consideration of the therapist’s time.

While some patient cancellations are inevitable, cancellations with less than 24 hours’ notice, missed appointments (no-shows) or being late for a scheduled appointment could result in the following:

Insurance:

As a courtesy to you, we will bill your insurance company for the services you receive at Encompass Physical Therapy. Any co-insurance, deductible or co-pays are due at the time of service. We also will verify your benefits for our services however, this is not a guarantee. It is ultimately your responsibility to be knowledgeable about your benefits package.
